| Sullivan EMS Interfacility Transport Service |
Background
Sullivan EMS began as a transporting agency on November 1, 2004, as an EMT-Basic
Service. Prior to November 2004, Sullivan Area First Responders (SAFR) provided first
responding care to the area until the transporting ambulance arrived on scene. SAFR
operated for approximately 20 years as a First Responding Agency.

On November 1, 2005, Sullivan EMS advanced the level of service to the Intermediate Technician level. In
July of 2006, Sullivan EMS acquired a second back up ambulance, which is a Ford F350 Southern Type III
ambulance. November of 2007, Sullivan EMS received the approval from the State of Wisconsin to provide
interfacility transfer services at the Intermediate Technician level.

Medical Direction
Dr. Timothy Westlake from Lake Country Emergency Physicians
(LCEP) is Sullivan EMS, LTD Medical Director. He works closely with
all the physicians from the group managing our medical direction.
Medical Control Hospital
Oconomowoc Memorial Hospital is the department’s medical control
hospital. The hospital is responsible for providing a physician on
call 24 hours-per-day and 7-days-per-week to furnish medical
information and on line direction.
Billing Procedures
Sullivan EMS’ billing agency is set up to provide assistance to any
agency that needs help. If there are any questions in regards to
